The novel coronavirus was likely able to remain so long in the woman's body because she had a compromised immune system due to previous treatment of lymphoma, a cancer in part of the immune system, Science Magazine reported. The patient had been successfully treated with CAR T-cell therapy about three years ago, which weakened her immune system by depleting her body of most of its B cells, immune system cells that make antibodies.

Many people will now be able to "mix and match" COVID-19 booster shots — that is, get a different COVID-19 vaccine for a booster, the Food and Drug Administration (FDA) announced Wednesday (Oct. 20).

미국 식품의약국(FDA)은

COVID-19 보조 주사제를

"섞어서 일치시킬" 수 있게 될 것,

Studies and real-world data have shown that this mix-and-match approach is safe and, in some cases, can even be more effective than not mixing. So should you get a booster vaccine that's different from your original dose? Live Science talked with a couple of experts, who agreed that mixing vaccines is perfectly safe, but their recommendations differed slightly.

The FDA has authorized booster doses for adults who are immunocompromised, who are 65 years or older or have underlying conditions, or those who are at high risk of exposure to COVID-19. People who received two shots of the Pfizer-BioNTech or Moderna vaccine are considered fully vaccinated and may be eligible for a booster at least 6 months from their last dose, whereas all adults who got the single-shot Johnson & Johnson vaccine are eligible to get a booster dose two months after their shot.

Data suggest that in people who received one of the two mRNA vaccines — that is, Pfizer or Moderna — getting a booster of the other mRNA vaccine will likely be equivalent to getting the same one. But in people who originally received the Johnson & Johnson vaccine, getting a Pfizer or Moderna booster may spur a much better immune response than receiving a second Johnson & Johnson shot.

Evidence on mixing

혼합에 대한 증거

The FDA's announcement to allow mixing and matching follows the early results of an ongoing National Institutes of Health (NIH) study. On Oct. 15, a group of researchers presented the results from the study, which hasn't yet been peer-reviewed and is posted as a preprint to medRxiv, to an FDA panel of experts.

The researchers tested nine different combinations of Johnson & Johnson, Moderna and Pfizer vaccines given to 458 participants and found that mixing was safe and highly effective. Receiving a booster shot greatly increased the circulating number of antibodies including neutralizing antibodies — molecules that bind to the virus and stop it from infecting cells — against SARS-CoV-2, the virus that causes COVID-19.

That's because "they're all looking fine with a boost," and there's more long-term data on the effectiveness of boosting with the same vaccine. What's more, some evidence suggests that mixing and matching these vaccines may lead to a broader immune response that may be better able to respond to a future SARS-CoV-2 variant, Malvestutto said. Still, many questions remain about how effective this approach is for other parts of the immune response.

"Immunity is not just about the levels of the antibodies; there are other parts to the story," Malvestutto said. The NIH study focused solely on the levels of antibodies. Another extremely important part of the immune response is what are known as memory cells, which circulate in the body long after neutralizing-antibody levels have depleted and prompt the immune system to make more targeted antibodies once exposed to the pathogen.

A mysterious condition known as "COVID toes" may finally have an explanation.

"COVID 발가락질환"으로 알려진

불가사의한 상태에

마침내

설명이 있게된 것이다.

It turns out that the frostbite-like rashes that appear on a person's toes after they catch COVID-19 may be a sign of a runaway immune response in which the body attacks its own tissues, a new study finds.

COVID-19에 걸린 후

새로운 연구 결과가 나왔다.

Cases of COVID toes started cropping up in the early days of the pandemic, and at the time, scientists had a few ideas as to why the condition manifested, Live Science previously reported. For instance, some dermatologists suggested that the telltale skin sores and bumps, known as pernio or chilblains, were caused by inflammation; others thought they might result from small blood clots in the toes.

COVID 발가락질환의 사례는

대유행 초기부터 나타나기 시작했고,

그것들은

생겼을 수도 있다고 생각했다.

Other viral diseases, such as measles, can cause skin rashes, so it wasn't necessarily surprising that COVID-19 could, too.; But now, the results of a new study, published Oct. 5 in the British Journal of Dermatology, hint at why the reddish-purple papules pop up on the toes and sometimes the fingers.

The researchers found that most of their study participants carried high levels of autoantibodies, proteins generated by the immune system that inadvertently attack the body's own tissues. They also found evidence that, compared with healthy individuals, the participants showed rampant activity of proteins called type 1 interferons, which switch on pathogen-fighting genes in immune cells. While these interferons help fight off viruses, human cells can end up suffering collateral damage in the process.

These findings fall in line with those of a very small study published in 2020 in the journal Dermatopathology, The New York Times reported. The study included only three patients, but it hinted that the trio developed toe rashes due to a strong interferon-driven immune response. This fiery immune response might help ward off the coronavirus, but it also may have resulted in the lingering rashes, the authors suggested in their report.

코로나 바이러스를 막는 데

도움이 될 수 있지만,

그것은 또한

지속되는 발진을

야기했을 수도 있다고

저자들은 보고서에서 제안했다.

At the time of the study, nearly 60% of the patients showed symptoms that were consistent with mild COVID-19, such as loss of smell or coughing, but all were negative for the virus on a PCR test. In general, COVID toes usually appear one to four weeks after a person contracts SARS-CoV-2,the virus that causes COVID-19, and the rashes can sometimes linger for months, The Guardian reported. So it's not surprising that someone would test negative for COVID-19 despite having COVID-related rashes on their feet, considering the rashes might emerge after the virus has been cleared from their system.

Besides the coronavirus, other triggers of chilblains include diseases like lupus, which none of the study participants had; and exposure to cold, which would be unlikely in springtime, the authors noted in their report. Given that cases of chilblain-like rashes ebbed and flowed with the waves of COVID-19 in 2020, overall, the available clues hint that their patients' rashes were linked to COVID-19 infection, the authors wrote in their report.

저자들은 보고서에서

They found a significant overlap in the results from presumed COVID-toe patients and the seasonal chilblain patients, in that both carried autoantibodies and showed signs of heightened type I interferon activity. Podiatrist Dr. Ivan Bristow told The Guardian that, for most people, COVID toes eventually clear up on their own, without the need for medication. That said, for those whose rashes are slow to heal, the new study could point toward helpful treatments, he said. For example, per the study's results, local or systemic anti-inflammatory agents, such as steroids, may be effective in treating COVID toes, The New York Times reported.

That said, data from the trial hasn't yet been peer-reviewed, so the drug's safety and efficacy still need to be confirmed.

The pill works by introducing mutations into the coronavirus's genetic material, or RNA, as the virus tries to make copies of itself, according to a report published Sept. 13 in the journal Nature Structural & Molecular Biology. In essence, once converted to its active form inside the body, the drug mimics a building block of RNA and wedges itself into the growing RNA sequence, thus gumming up the works. This antiviral mechanism is somewhat unusual, so safety data from the trial will need to be carefully assessed to see if the pill has any unintended effects, Dr. Andy Pavia, chief of the Division of Pediatric Infectious Diseases at University of Utah, told Stat News.

"With the virus continuing to circulate widely, and because therapeutic options currently available are infused and/or require access to a healthcare facility, antiviral treatments that can be taken at home to keep people with COVID-19 out of the hospital are critically needed," Wendy Holman, chief executive officer of Ridgeback Biotherapeutics, said in the Merck statement.

The trial included 775 adults diagnosed with mild-to-moderate COVID-19, and each of the participants had at least one risk factor associated with poor COVID-19 outcomes, such as diabetes or heart disease. Within five days of developing symptoms, the participants were randomly assigned to receive either a five-day course of molnupiravir or a placebo pill, Merck reported. Participants took two pills a day over the five days.

The Merck statement included little information about side effects, but it stated that the rate of "adverse events" was similar between the treatment and placebo groups, Stat News reported. Specifically, 35% of those who received molnupiravir and 40% of those who received a placebo experienced an adverse event of some kind; in total, 1.3% of the molnupiravir-treated participants stopped taking the pill due to these effects, as compared with 3.4% who stopped taking the placebo.

Based on the company, it's unclear whether these adverse events were related to the pills themselves or just symptoms of COVID-19. Merck's head of research and development Dean Li, told Stat News that, in this early assessment, no particular side effect stood out as being more common with molnupiravir, but more detailed data will be available in the future.

Based on these positive results, an independent Data Monitoring Committee and the FDA advised the drugmakers to stop recruiting new participants to the trial, because at this point, giving COVID-19 patients a placebo instead of the pill would be unethical, according to the Merck statement.

"The fact that the new clinical trial was stopped by the Data and Safety Monitoring Board early because of overwhelming efficacy, deeming it unethical to proceed, would be considered a Eureka moment in the fight against Covid," Dr. Eric Topol, a professor of molecular medicine at Scripps Research who has served on multiple FDA advisory committees, wrote in a tweet.

Merck and Ridgeback Biotherapeutics plan to submit an application for Emergency Use Authorization (EUA) to the FDA "as soon as possible," and will also apply for clearance from regulatory bodies in other countries, according to the company statement. Merck said it can produce 10 million courses of molnupiravir by the end of 2021, and in June, the U.S. government purchased 1.7 million courses.
